Prior to survey administration, each institution must submit all of the following items to COACHE:
A signed participation agreement reflecting your institution’s data disclosure preference. Will you receive just the COACHE report (Data Option 1), or do you wish to receive also a record-level data file (Data Option 2)?
Note that there could be a tradeoff between the level of data disclosed to your institution and the degree of participation to which your junior faculty will consent.
Payment of the first installment of the enrollment fee. Checks should be made payable to “Harvard University”
and sent to the Collaborative at the address below.
A database of your eligible faculty in the format described in the attached instructions.
Optional: Custom questions, if your institution wishes to append any additional 5-point-scale or open-ended questions not appearing in the core COACHE survey (additional fees apply). Sample questions are available upon request.
A copy of the letter of introduction to be sent to eligible faculty by your institution’s president, chief academic
officer, or other senior representative. The letter should be sent approximately one week prior to survey launch. Sample letters are available for your use.
